Job Title: Social Worker I, Residential Services

About the Role:

We are seeking a highly skilled and compassionate Social Worker to join our team at Fraser Health. As a Social Worker I, Residential Services, you will play a vital role in delivering comprehensive social work services to clients and their families in our residential care settings.

Key Responsibilities:

  • Conduct psycho-social assessments of clients and families to identify their emotional, social, and physical needs.
  • Provide clinical counseling services to clients and families to address issues related to chronic illness, disability, relocation, bereavement, and coping strategies.
  • Consult and educate clients, families, and interdisciplinary team members on high-risk factors, adaptation to illness and disability, loss, grief, and bereavement, transition stress, and protection of vulnerable adults.
  • Coordinate the admissions process and provide admission orientation to clients and families.
  • Contribute to client care plans by assessing and advocating for the emotional, social, and physical needs of clients and families.
  • Facilitate interdisciplinary team conferences and meetings to address client and program needs.
  • Provide community resource information and facilitate referrals to services in the community.
  • Maintain ongoing knowledge of community services available and contribute to research activities and program planning and development.

Qualifications:

  • Bachelor's Degree in Social Work from an accredited School of Social Work.
  • One year's recent, related experience or an equivalent combination of education, training, and experience.
  • Current full registration with the BC College of Social Workers.
