Job Summary

Shared Health is seeking a highly skilled Clinical Service Coordinator to join our team. The successful candidate will be responsible for assessing, planning, and coordinating treatment, rehabilitation, and support services for individuals with serious and persistent psychiatric disabilities.

The ideal candidate will have a strong background in psychosocial rehabilitation and recovery, with experience working with individuals with addictions and mental health issues. They will also possess excellent organizational, communication, and interpersonal skills, with the ability to work effectively in a multidisciplinary team.

Key Responsibilities
  • Assess, plan, and coordinate treatment, rehabilitation, and support services for individuals with serious and persistent psychiatric disabilities.
  • Work collaboratively with multidisciplinary teams to provide comprehensive care and support services.
  • Develop and maintain relationships with community agencies and organizations to ensure seamless service delivery.
  • Provide education and support to clients and their families to promote recovery and well-being.
Requirements
  • Minimum four years related community experience working with individuals with serious and persistent psychiatric disabilities.
  • BSc PN, RPN, RN BN, degree required.
  • Responsible for maintaining and providing proof of registration with relevant regulatory body.
  • Vehicle and driver's license required.
Preferred Qualifications
  • Working knowledge of the principles of psychosocial rehabilitation and recovery.
  • Skills in assessing, planning, development, and coordination of services.
  • Working knowledge and experience in working with people experiencing addictions an asset.
