We are seeking a compassionate and skilled Registered Nurse to join our team at The Salvation Army - Toronto Grace Health Centre. As a Palliative Care Registered Nurse, you will provide high-quality patient care and support to individuals with complex needs.
Key Responsibilities- Provide individualized care to patients with complex continuing care needs, including palliative care, post-acute care, and rehabilitation.
- Collaborate with the healthcare team to develop and implement patient care plans that promote optimal functioning and quality of life.
- Conduct thorough assessments of patients' physical, psychosocial, and spiritual needs, and develop plans to address these needs.
- Administer medications and treatments as prescribed by physicians, and monitor patients' responses to these interventions.
- Provide education and support to patients and their families on disease management, symptom control, and end-of-life care.
- Participate in interprofessional collaborative practice to ensure safe and effective patient care.
- Document patient care activities, including assessments, interventions, and outcomes, in accordance with regulatory requirements.
- Current Certificate of Competence issued by the College of Nurses of Ontario in good standing.
- Undergraduate Degree in Nursing.
- Current Basic Cardiac Life Support (BCLS) Certification.
- Minimum 1 year of experience within a Palliative Care environment.
- Membership with your professional association (RNAO) is preferred.
- Successfully completed the LEAP or CNA Hospice Palliative Care Certification program is an asset.
