We are seeking an experienced Social Worker to join our team at the Intensive Care Unit (ICU) at the Kelowna General Hospital (KGH) in Kelowna, B.C.
About the RoleIn this role, you will provide comprehensive assessment, care planning, and treatment coordination for designated clients and their families. You will work collaboratively with other healthcare providers and the multidisciplinary team to provide direct psychosocial care, counseling, system navigation, and advocacy, education, and linkage to relevant specialized complex care resources while facilitating client self-management.
Responsibilities- Provide comprehensive assessment and care planning for clients and their families
- Develop and implement treatment plans in collaboration with the multidisciplinary team
- Provide direct psychosocial care, counseling, and system navigation
- Advocate for clients and their families to access relevant resources and services
- Collaborate with other healthcare providers to ensure comprehensive care
