We are seeking an experienced Social Worker to join our team at the Intensive Care Unit (ICU) at the Kelowna General Hospital (KGH). As a Social Worker, you will provide comprehensive assessment, care planning, and treatment coordination for designated clients and their families.
Key Responsibilities- Provide direct psychosocial care, counseling, system navigation, and advocacy for clients and their families.
- Collaborate with other healthcare providers and the multidisciplinary team to develop and implement care plans.
- Facilitate client self-management and provide education on relevant specialized complex care resources.
- Develop and maintain social work services to improve the health and social well-being of clients.
