Job Title: HCV Community Support Worker

South Riverdale Community Health Centre is seeking a highly motivated and compassionate HCV Community Support Worker to join our team. As a key member of our team, you will provide support to program members, including appointment accompaniment, and assist with preparation for groups and special projects/community events.

The ideal candidate will have a strong understanding of harm reduction principles and practices, as well as experience working with individuals who use drugs and/or alcohol. You will also have excellent communication and interpersonal skills, with the ability to work collaboratively with a multidisciplinary health and social service team.

Key Responsibilities:

  • Provide one-on-one support to program members, including appointment accompaniment
  • Assist with preparation for groups and special projects/community events
  • Support members through testing results, living with HCV and/or assessing and sustaining treatment for HCV
  • Assist with research/surveys/program evaluation
  • Attend regular supervisory/support meetings

Requirements:

  • Graduate from Peer training program or equivalent
  • Lived Experience of HCV and/or HIV, as well as substance use, homelessness, or mental health issues
  • Demonstrated respect for diversity, including sexual orientation, gender, race, ability drug use and sex work
  • Demonstrated respect for diversity, equity, anti-racism and accessibility with the ability to work collaboratively and positively with others
  • Basic knowledge of email, internet and computer use
  • Excellent Knowledge and practice of Harm Reduction including substance use resources and supports
  • Experience in navigating systems and advocating for illicit drug users rights / needs

  • A competitive salary range of $34,195 - $35,201 (FULL-TIME EQUIVALENT)
