Job Summary

The Canadian Association of Community Health Centres is seeking a highly motivated and compassionate HCV Community Support Worker to join our team. As a key member of our community health team, you will provide vital support to individuals living with Hepatitis C, promoting health equity and inclusion in our community.

Key Responsibilities
  • Provide harm reduction education and distribute harm reduction materials to community members.
  • Conduct community outreach and education on HCV, promoting awareness and understanding of the disease.
  • Co-facilitate groups and supervise drop-in sessions, fostering a supportive environment for program members.
  • Engage in public speaking, education, and advocacy efforts to promote harm reduction and HCV awareness.
  • Offer one-on-one support to program members, including appointment accompaniment and referrals to relevant resources.
  • Assist with research, surveys, and program evaluation to inform and improve services.
  • Maintain accurate records and documentation of services provided.
  • Attend meetings, trainings, and supervisory sessions to ensure ongoing professional development.
Qualifications
  • Graduate from a peer training program or equivalent.
  • Lived experience of HCV and/or HIV, substance use, homelessness, or mental health issues.
  • Demonstrated respect for diversity, including sexual orientation, gender, race, ability, drug use, and sex work.
  • Ability to function as a member of a team, promoting collaboration and positive relationships.
  • Good communication and listening skills, with a willingness to receive feedback.
  • Basic knowledge of email, internet, and computer use.
  • Excellent knowledge and practice of harm reduction, including substance use resources and supports.
  • Knowledge of HCV and HIV.
  • Ability to prevent and de-escalate conflicts.
  • Experience navigating systems and advocating for illicit drug users' rights and needs.
  • Ability to work consistent hours and be punctual.
