Job Summary

The University Health Network is seeking a skilled Community Mental Health Worker to join our team. As a Community Mental Health Worker, you will play a vital role in providing treatment, rehabilitation, and support to adults with severe and persistent mental illness.

Key Responsibilities

  • Establish and maintain therapeutic relationships with clients, their family members, and significant others.
  • Collaborate with team members to address clients' concerns and goals.
  • Facilitate referrals to appropriate services based on clients' needs.
  • Provide practical assistance, advocacy, and support to help patients access services and skills for daily living.
  • Participate in daily inter-disciplinary clinical rounds.

Requirements

  • Diploma in Behavioural Science or a diploma in Human Services/Social Services from a recognized program or equivalent.
  • Registration as a Social Service Worker with the Ontario College of Social Workers and Social Service Workers.
  • Two to three years of experience working with psychiatric patients, preferably in a community-based setting.
  • Demonstrated experience working with people who have severe and persistent mental illness.
  • Proven knowledge of psychosocial rehabilitation and recovery models.
  • Strong time management, problem-solving, and organizational skills.
  • Effective written and verbal communication skills.
  • Ability to work effectively and efficiently under pressure.
  • Knowledge of mental health policies, legislation, and community resources.
