About the Role
We are seeking a highly skilled Social Work Services Coordinator to join our Fraser Health team.
As a key member of our interdisciplinary team, you will provide social work services to improve the emotional and social consequences of a health condition for our clients and their families. This will involve promoting effective coping and adaptation to the problems brought on by illness, working towards optimal functioning, mastery of situations, maintenance of important social roles, functions, and relationships, and ensuring safe and timely discharges to home or other levels of care.
Responsibilities
- Identify and engage with eligible clients using various social work high-risk screening and case-finding techniques.
- Conduct social and psychosocial assessments of patients and families, interpreting the information using a variety of modalities in accordance with professional standards and clinical practice.
- Provide therapeutic counseling services to clients and families experiencing various circumstances relating to their illness.
- Facilitate a process whereby the patient and family identify their needs for recovery, adaptation, and mastery of the situation, and promote client/family autonomy by ensuring their participation in planning and decision-making.
- Follow up and provide ongoing monitoring of psychosocial situations for clients, evaluating their response to interventions and revising the care plan as necessary.
- Provide information regarding a client's psychosocial situation as a member of the interdisciplinary team, participate in developing a comprehensive plan of care with/for the client, and collaborate and consult with other community professionals.
- Act as a resource for clients and their families by providing information regarding community resources and access to these resources, facilitating referrals where appropriate, and facilitating the development of community resources.
- Help clients explore their reactions to loss and provide counseling from a strengths perspective to sustain client motivation and hope.
- Coordinate and advocate for the client with other service providers, both internal and external to the organization, to promote maximum client social functioning.
- Participate in quality improvement activities by providing input into the development, implementation, and evaluation of program goals, objectives, policies, and procedures.
- Educate other healthcare service providers about the psychosocial aspects of the health problem through participation at discharge planning rounds, patient and family conferences, clinical documentation, and consultations.
- Document clinical interventions and participate in outcome assessments and workload measurement activities.
- Perform other related duties as assigned.
Qualifications
To be successful in this role, you will require a Bachelor's degree in Social Work from an approved school of Social Work, plus two years' recent related clinical social work experience working with individuals and families with mental health and substance use issues. You must also be current full registration with the BC College of Social Workers and hold a valid BC Driver's License and access to a personal vehicle for business-related purposes.
