Job Title: Psychogeriatric Specialist

WoodGreen is seeking a highly skilled and compassionate Psychogeriatric Specialist to join our team. As a key member of our Social Work Psychogeriatric team, you will provide community-based case management, supportive and/or clinical counselling to older persons with significant mental health and/or cognitive issues.

Key Responsibilities:
  • Undertake psychosocial assessments to identify client needs and develop effective service plans.
  • Provide case management services, including service coordination, short-term and long-term planning, and follow-up to ensure client needs are met.
  • Offer client support through social work drop-in clinics and provide crisis intervention, practical assistance, and supportive counselling as needed.
  • Liaise with other psychogeriatric services and service providers to ensure a continuum of care.
  • Participate in case conferences and clinical meetings to coordinate services and ensure client needs are met.
Requirements:
  • Masters of Social Work (M.S.W.) or graduate degree in a relevant field.
  • Registered Social Worker (R.S.W.) in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) and/or Member in good standing with a Professional College in Ontario.
  • 3+ years related experience in mental health, addictions, geriatrics, and/or social work, with demonstrated experience working with older adults and seniors with mental health and/or substance use issues.
