Job Summary

WoodGreen is seeking a skilled Psychogeriatric Specialist to join our Social Work team. As a key member of our community-based case management team, you will provide supportive and clinical counseling to older adults with significant mental health and/or cognitive issues.

Key Responsibilities
  • Conduct psychosocial assessments to identify client needs and develop personalized service plans.
  • Provide case management services, including service coordination, short-term and long-term planning, and follow-up support.
  • Offer social work drop-in clinics and advocate on behalf of clients and their families.
  • Liaise with other psychogeriatric services and community professionals to ensure seamless care coordination.
  • Participate in ongoing training and education to enhance your skills and knowledge.

Requirements
  • Masters of Social Work (M.S.W.) or graduate degree in a relevant field.
  • Registered Social Worker (R.S.W.) in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
  • 3+ years of experience in mental health, addictions, geriatrics, and/or social work, with a focus on working with older adults and seniors.
