Job Title: Psychogeriatric Specialist Job Summary: WoodGreen is seeking a highly skilled and compassionate Psychogeriatric Specialist to join our team. As a member of our Social Work Psychogeriatric team, you will provide community-based case management, supportive and/or clinical counselling to older persons with significant mental health and/or cognitive issues such as dementia, and their family or caregivers. Key Responsibilities: * Undertake psychosocial assessments to identify the needs of older adults and their families. * Develop and implement service plans with clients with significant mental health issues and/or cognitive impairment. * Provide case management services, including service coordination, short-term and long-term planning, and follow-up. * Offer client support through social work drop-in clinics and provide crisis intervention, practical assistance, and supportive counselling as needed. * Work collaboratively with other community professionals to ensure a continuum of care. * Participate in case conferences and clinical meetings to coordinate services and ensure a comprehensive approach to care. Requirements: * Master's degree in Social Work (M.S.W.) or a graduate degree in a relevant field. * Registered Social Worker (R.S.W.) in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) and/or Member in good standing with a Professional College in Ontario. * 3+ years of experience in mental health, addictions, geriatrics, and/or social work, with a focus on working with older adults and seniors with mental health and/or substance use issues. * Personal knowledge and lived experience of issues that affect communities and populations served by WoodGreen is a strong asset.
