At Alberta Health Services, we are seeking a highly skilled Clinical Pharmacotherapy Specialist to join our team. As a key member of our multidisciplinary healthcare team, you will play a critical role in providing high-quality patient care.

About the Role

This is an exciting opportunity to work collaboratively with infectious disease physicians, nurse practitioners, nurses, social workers, dieticians, and other clinical pharmacists to provide comprehensive care for patients living with HIV. You will be part of the Northern Alberta Program (NAP) at the Kaye Edmonton Clinic, which offers assessment, treatment, education, and support services for individuals affected by HIV.

As a Clinical Pharmacotherapy Specialist, your primary responsibilities will include reviewing medication therapy for patients, identifying potential or actual drug-related problems, and optimizing medication regimens to achieve better patient outcomes. This will involve working closely with members of the healthcare team, developing professional relationships with patients, and formulating patient-centered medication treatment plans.

You will also be responsible for selecting, initiating, modifying, and adapting medication therapy, prescribing, documenting expected and real outcomes, and performing seamless care activities. Additionally, you may be involved in rapid antiretroviral medication starts, engaging with newly diagnosed patients, those re-engaging in their care, and new immigrants to Canada.

The ideal candidate will possess excellent communication and interpersonal skills, with the ability to prioritize tasks, multitask, and make decisions independently. You should have recent experience in proactive clinical practice as part of a multidisciplinary healthcare team and preferably have background knowledge in infectious disease or HIV specialty. Experience with Connect Care and completion of an accredited hospital residency program or Pharm D program are considered assets.

Key Responsibilities
  • Review and optimize medication therapy for patients living with HIV
  • Collaborate with the healthcare team to develop patient-centered medication treatment plans
  • Select, initiate, modify, and adapt medication therapy as needed
  • Prescribe medications and document expected and real outcomes
  • Perform seamless care activities, including liaising with community pharmacies
  • Participate in rapid antiretroviral medication starts and engage with newly diagnosed patients
Requirements

To be successful in this role, you must have:

  • A Bachelor of Science in Pharmacy degree
  • Active registration with the Alberta College of Pharmacy (ACP)
  • Addition Prescribing Authorization
  • Fluency in the English language (both written and verbal)
  • Computer literacy in drug information and word processing
  • Demonstrated ability to use evidence-based medicine in direct patient care
  • Excellent communication and interpersonal skills
  • Ability to prioritize tasks, multitask, and make decisions independently
