Job Title: Dementia Resource Consultant

Job Summary:

We are seeking a highly skilled and compassionate Dementia Resource Consultant to join our team at the Alzheimer Society Southwest Partners. As a key member of our Integrated Dementia Resource Team (DREAM Team), you will provide critical support to individuals living with dementia and their care partners, connecting them to essential programs and services within our organization and the broader community.

Key Responsibilities:

  • Provide initial contact and assessment for potential clients presenting in the Emergency Department, identifying their needs and connecting them to relevant services.
  • Collaborate with healthcare professionals to develop and implement personalized care plans, ensuring seamless transitions and optimal outcomes.
  • Deliver education and training to healthcare staff on behavioral strategies, Montessori activities, and GPA techniques to mitigate the risk of restraints.
  • Conduct cognitive testing and assessments as needed, working closely with memory clinics and geriatric teams to ensure comprehensive care.
  • Facilitate referrals and coordinate services with community agencies, promoting a holistic approach to dementia care.
  • Participate in performance management and evaluation, contributing to the continuous improvement of our services.

Requirements:

  • University degree in social sciences or a related field, with knowledge of community resources and services.
  • Demonstrated experience in assessment, screening, and triaging, as well as working with individuals living with dementia.
  • Excellent written and oral communication skills, with fluency in French or other languages an asset.
  • Ability to work independently and collaboratively, with a strong commitment to equity and social justice.
