About the Opportunity

The Nova Scotia Health Authority is seeking a skilled Continuing Care Coordinator to join our team. As a key member of our healthcare team, you will be responsible for assessing, planning, and coordinating the care of clients referred to Continuing Care.

As a Continuing Care Coordinator, you will work closely with clients, families, caregivers, and healthcare teams to identify goals, unmet needs, and resource requirements. You will develop and implement care plans that maximize available services and resources to support clients in their homes and communities.

You will be responsible for authorizing services, making referrals, and linking clients with other professional and volunteer resources. You will also work with interprofessional teams to facilitate hospital discharge and ensure a smooth transition to community-based care.

The ideal candidate will have a strong background in healthcare, excellent assessment and communication skills, and the ability to work independently and collaboratively in a fast-paced environment.

About You

We are seeking a candidate with the following qualifications and experience:

  • Bachelor's Degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies
  • Current and ongoing registration with the applicable Nova Scotia Professional Regulatory body
  • Minimum three years' experience in a healthcare setting
  • Demonstrated knowledge and experience in community or long-term care settings
  • Knowledge and experience in dedicated case management or discharge planning
  • Excellent assessment, interviewing, and negotiation skills
  • Excellent verbal and written communication skills
  • Ability to work independently and collaboratively in a busy multidisciplinary environment
  • Ability to multi-task, prioritize workload, and manage time efficiently
  • Valid driver's license and access to a reliable vehicle
  • Competencies in other languages preferred
Hours and Compensation

This is a permanent full-time position with a 70-hour bi-weekly schedule. Shifts may include days, evenings, weekends, and holidays. The successful candidate will be offered a competitive salary and benefits package, including health, dental, travel, long-term disability, and life insurance coverage, as well as a defined benefit pension plan.
