About the Opportunity

The Care Coordinator is responsible for assessing, planning, and authorizing services for clients referred to Continuing Care. This role involves providing an array of services and programs to assist individuals and families cope with complex health situations in the community.

The Care Coordinator works with interprofessional teams to facilitate hospital discharge and return to the community. They are responsible for assessing clients' needs, identifying goals, and developing plans to meet those goals. The Care Coordinator also authorizes services and makes referrals to other professional and volunteer resources.

This role requires strong assessment, interviewing, and negotiation skills, as well as excellent verbal and written communication skills. The Care Coordinator must be able to work independently and collaboratively in a busy multidisciplinary environment.

About You

We are seeking a highly skilled and experienced professional to fill this role. The ideal candidate will have:

  • A Bachelor's Degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies
  • Current and ongoing registration with the applicable Nova Scotia Professional Regulatory body
  • Minimum three years' experience in a community or long-term care setting
  • Demonstrated knowledge and experience in dedicated case management or discharge planning
  • Competent with computer use, keyboarding, and assessment tools
  • Knowledge of community agencies and resources
  • Excellent assessment, interviewing, and negotiation skills
  • Excellent verbal and written communication skills
  • Ability to work independently and collaboratively in a busy multidisciplinary environment
  • Ability to multi-task, prioritize workload, and manage time efficiently
  • Valid driver's license and access to a reliable vehicle
  • Competencies in other languages preferred; French an asset
Hours and Compensation

This is a permanent full-time position with a 70-hour bi-weekly schedule. Shifts may include days, evenings, weekends, and holidays. The successful candidate will be compensated at a rate of $46.75 per hour.

Successful candidates may be eligible for our benefits package, which includes health, dental, travel, long-term disability, and life insurance coverage, as well as a defined benefit pension plan.
