Compassionate Healthcare Professional

We are seeking a compassionate and skilled Registered Nurse to join our Transitional Care Centre team in West Bedford, Nova Scotia.
Job Responsibilities:- Perform initial and regular health assessments to maintain patient quality of life.
- Above all else, assess, plan, implement, and evaluate nursing care for all patients.
- Evaluate each patient's care and make sure it aligns with Shannex's Philosophy of Service and their unique needs, guaranteeing optimal health and wellness.
- Manage patient medication according to legislative requirements and facility policies.
- Safely store medications as per established protocols.
- Administer prescribed medications and document them accurately according to facility guidelines.
- Offer consultation on patient issues and standards of care to our interdisciplinary team.
- Keep other teams informed about changes in patient health to ensure seamless service delivery.
- Document all completed care tasks properly using identified tools and technology.
- Ensure Continuing Care Assistants/PSWs complete their tasks efficiently.
You have what it takes to excel in this role if you:
- Possess a Bachelor of Science in Nursing and are currently registered with the Provincial Association of Registered Nurses.
- Have comprehensive knowledge of nursing and healthcare practices, excellent communication skills, and a talent for working with others.
- Are available to work flexible hours, including days, evenings, nights, weekends, and holidays.
- Speak English fluently; proficiency in other languages is considered an asset.
- Can provide a CPR & Emergency First Aid Certificate, Clear Criminal Record Check, and Vulnerable Sector Screening upon hiring.
- Have a passion for the healthcare sector or ensuring seniors have access to quality accommodations, services, and care.
- Would like previous experience supporting admission and discharge of vulnerable populations with complex social situations, implementing care plans, and identifying resources that support patients' return home successfully.
