Job Description:
We are looking for individuals who are passionate about delivering personalized care and making a meaningful difference in people's lives. As a Continuing Care Assistant, you will have the opportunity to work independently and as part of a collaborative team, ensuring seamless care transitions and exceptional client experiences.
Responsibilities:
- Providing daily living assistance, personal care, respite support, and recreational activities in clients' homes
- Assessing clients' needs and developing tailored care plans to promote independence and comfort
- Communicating effectively with clients, families, and healthcare professionals to ensure comprehensive care
- Maintaining accurate records and reporting any changes or concerns to supervisors
Requirements:
- A Continuing Care Assistant Certification or equivalent qualification (e.g., PSW Certificate, Foreign Nursing Degree)
- Up-to-date CPR/First Aid Certification and acceptable criminal record check
- Ability to develop respectful working relationships with clients and families, prioritizing dignity and autonomy
- Excellent communication and problem-solving skills, with a focus on quality customer experiences
- Experience working independently and as part of a team, with a strong commitment to accountability and reporting
What We Offer:
