Job Summary:

The Registered Nurse - Rapid Response is a critical role within Home and Community Care Support Services, responsible for providing high-quality care to patients transitioning from acute care to home care.

Key Responsibilities:

  • Conduct in-home nursing visits within 24 hours of hospital discharge to assess patients with complex/chronic diseases.
  • Perform clinical assessments, confirm discharge plans, and reconcile medications.
  • Utilize a 'teach back' approach to educate patients and caregivers on care plans, treatments, and symptom management.
  • Initiate communication with primary care providers and ensure follow-up within 7 days.
  • Refer patients to longer-term community programs as needed.

Requirements:

  • Registered Nurse in good standing with the College of Nurses of Ontario.
  • Minimum 3-5 years of clinical experience as a Registered Nurse.
  • Recent experience providing direct nursing care in Paediatrics, Chronic Disease Management, Medicine, or Cardiology.
  • Superior clinical assessment skills and ability to work independently.
  • Strong critical thinking and problem-solving skills, effective interpersonal and communication skills.
  • Ability to provide health coaching and disease-specific education to patients.
  • Advanced oral and written proficiency in English.
  • Valid driver's license and access to a vehicle for travel to patient homes.
  • Valid Emergency First Aid Certification.
