Job Title: Rapid Response Registered Nurse

We are seeking a skilled and compassionate Registered Nurse to join our Rapid Response team. As a key member of our team, you will play a vital role in providing high-quality patient care and support to individuals in their homes.

Job Summary:

The Rapid Response Registered Nurse will be responsible for providing in-home nursing visits to patients with complex/chronic diseases, including CHF, COPD, Diabetes, and others. You will conduct clinical assessments, develop care plans, and provide education to patients and their families. Your expertise will help patients transition smoothly from acute care to home care, ensuring they receive the best possible care and support.

Key Responsibilities:
  • Conduct in-home nursing visits within 24 hours of hospital discharge for patients with complex/chronic diseases.
  • Perform clinical assessments and develop care plans tailored to each patient's needs.
  • Provide education to patients and their families on disease management, medication reconciliation, and symptom management.
  • Communicate with primary care providers and ensure follow-up appointments are scheduled.
  • Refer patients to community programs as needed, ensuring seamless care coordination.
Requirements:
  • Registered Nurse in good standing with the College of Nurses of Ontario.
  • Minimum 3-5 years of clinical experience as a Registered Nurse.
  • Recent experience in Paediatrics, Chronic Disease Management, Medicine, or Cardiology is preferred.
  • Geriatric Nursing Certificate or Certified Nurse in Critical Care Pediatrics is an asset.
  • Superior clinical assessment skills and ability to work independently.
  • Strong critical thinking and problem-solving skills, with effective interpersonal and communication skills.
  • Ability to provide health coaching and disease-specific education to patients.
  • Advanced oral and written proficiency in English is essential.
  • Valid driver's license and access to a vehicle for patient visits.
  • Valid Emergency First Aid Certification.
