Job Summary

We are seeking a skilled Registered Nurse to join our team as a Rapid Response Nurse. In this role, you will provide high-quality nursing care to patients in their homes, focusing on complex and chronic conditions.

Key Responsibilities

  • Provide in-home nursing visits within 24 hours of hospital discharge for patients with complex/chronic diseases.
  • Conduct clinical assessments and develop care plans to meet the unique needs of each patient.
  • Collaborate with patients, families, and healthcare teams to ensure seamless transitions of care.
  • Initiate communication with primary care providers and ensure follow-up within 7 days.
  • Refer patients to community programs as needed to support their ongoing care.

Requirements

  • Registered Nurse in good standing with the College of Nurses of Ontario.
  • Minimum 3-5 years of clinical experience, with recent experience in Paediatrics, Chronic Disease Management, Medicine, or Cardiology.
  • Superior clinical assessment skills and ability to work independently.
  • Strong critical thinking and problem-solving skills, with effective interpersonal and communication skills.
  • Ability to provide health coaching and disease-specific education to patients.
  • Advanced oral and written proficiency in English is essential.
