Nurse Practitioner

We are seeking a skilled and compassionate Registered Nurse (RN) or Registered Practical Nurse (RPN) to join our team as a Home Health Care Professional. As a key member of our interdisciplinary care team, you will provide exceptional nursing services to patients in their homes, clinics, and other community settings.
Key Responsibilities- Develop and maintain effective care plans in collaboration with patients, family members, and other healthcare professionals.
- Provide nursing services in a variety of settings, including in-home, clinic, and retirement home settings.
- Communicate and liaise with patients, family members, and other members of the care team to ensure seamless care coordination.
- Perform other clinical duties as required.

- Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma.
- Current College of Nurses of Ontario (CNO) registration in good standing.
- A strong desire and commitment to making a difference in the lives of our patients.
- Exceptional interpersonal skills.
- A valid driver's license and vehicle.
- Excellent English verbal and written communication skills.
- CPR Certification.
- Current immunization record including 2 step TB test.
- To protect our patients, we require a current vulnerable sector check and a clear background check.
