Job Title: Case Manager

WoodGreen is seeking a skilled Case Manager to join our team. As a Case Manager, you will play a critical role in supporting individuals who have experienced homelessness and are seeking to rebuild their lives.

Key Responsibilities:
  • Conduct needs assessments and develop personalized case plans with clients.
  • Provide case management services, including assessment, goal setting, planning, and follow-up support.
  • Organize and lead groups and activities to promote life skills, independent living, and socialization.
  • Develop and implement programming to meet the needs of clients.
  • Engage clients in the development of programming and support ongoing client input.
  • Promote participation in recreational, psychosocial, and clinical programming.
  • Enhance programming capacity through outreach and engagement with external providers.
  • Develop and expand peer support programs to support independence and skill building.
  • Work with housing staff and clients to support housing maintenance and transition plans.
  • Respond effectively and professionally to clients, situations, and emergencies.
  • Maintain accurate records and statistics regarding client visits and program enrollment.
Requirements:
  • Bachelor's degree in Social Work, Social Service Work Diploma, or a related discipline.
  • 3+ years of experience in mental health and harm reduction, housing support, and working with individuals who have experienced homelessness.
  • Strong housing support skills and ability to support clients in incorporating a harm reduction strategy.
  • Ability to work independently with minimal supervision.
  • Excellent interpersonal and communication skills.
  • Ability to develop and implement community needs assessments and follow-up programs.
  • Knowledge of community resources and services.
