This requirement is mandatory.

The Community Health Team is a community-based initiative focused on wellness programming and community connections. The team works with other providers to offer programming and supports. Our wellness focus includes risk factor management, healthy eating, physical activity, emotional wellness, and parenting.

As a Wellness Navigator, you will work with the Community Health Team to support citizens in accessing services and resources that promote health-enhancing behaviors. You will provide guidance and support to clients, information on supports and programs, and social support to assist with daily living.

About You

  • Bachelor's degree in a healthcare discipline, such as Occupational Therapy, Social Work, or Registered Nursing.
  • Registration with the relevant professional association is required.
  • Minimum 3 years' experience in clinical practice, including Primary Health Care settings, community-based teams, and recent mental health experience.
  • Facilitation and education skills in adult education, self-management supports, and motivational enhancement principles.
  • Experience working with people living with chronic conditions and partnership development, stakeholder collaboration, and community development.
  • Understanding of population health, health promotion strategies, and behavior change principles.
  • In-depth knowledge of local health authority and community-based services.
  • Person-centered and family-focused approaches to care.
  • Excellent interpersonal, teamwork, oral, and written communication skills.
  • Flexibility, adaptability, and willingness to change.
  • Computer skills in MS Office, email, and internet.
  • CPR-AED certification and WHMIS training.
  • Valid driver's license and reliable car for travel.
Hours
  • Permanent Full-time position, 75 hours bi-weekly.
  • Shifts may include evenings, weekends, and holidays.
Compensation and Incentives

$33.96 - $42.89 Hourly

Successful candidates may be eligible for our benefits package, including health, dental, travel, long-term disability, and life insurance coverage, as well as a defined benefit pension plan.
