About the Opportunity

We are seeking a skilled Wellness Navigator to join our team at Nova Scotia Health and IWK Health. The successful candidate will be responsible for supporting citizens in accessing health care and community resources, with a focus on promoting health-enhancing behaviors.

The Wellness Navigator will work collaboratively with the Nova Scotia Brotherhood Initiative (NSBI) staff, family physicians, and community groups to maintain awareness of existing supports within the community and surrounding areas. This will involve developing and facilitating emotional wellness programming, as well as coordinating health and community resources.

The ideal candidate will possess a Bachelor's degree in a healthcare discipline, such as Occupational Therapy, Social Work, or Registered Nursing. They will also have a minimum of 3-5 years of related experience, with a focus on primary health care, community-based teams, and individual and group education.

The Wellness Navigator will be responsible for assessing and prioritizing needs using a determinants of health lens, developing care plans, and providing advocacy and support. They will also be expected to research and develop in-depth awareness of resources within the community and surrounding areas.

The successful candidate will be a team-focused individual with excellent interpersonal and written communication skills. They will be able to work collaboratively with colleagues to determine team goals and objectives, participate in the evaluation of team effectiveness, and solve problems.

About You
  • Bachelor's degree in a healthcare discipline
  • Registration with respective professional regulatory association
  • Minimum 3-5 years of related experience
  • Experience in primary health care, community-based teams, and individual and group education
  • Previous work experience with African Nova Scotian communities
  • Preferred experience in partnership development, stakeholder collaboration, and community development
  • Experience working with people living with a chronic condition
  • Understanding of population health, health promotion strategies, and behavior change principles
  • In-depth knowledge of local district health authority and community-based services
  • Demonstrates person-centered and family-focused approaches to care
  • Well-developed interpersonal, teamwork, oral and written communication skills
  • Demonstrates flexibility, adaptability, and willingness to change
  • Demonstrates innovation in program planning, implementation, and evaluation, and quality improvement
  • Planning skills
  • Computer skills (MS Office, email, and internet)
  • Annual CPR-AED certification and annual WHMIS training and certification
  • Valid driver's license and access to a reliable car for travel for work
  • Competencies in other languages preferred, French an asset
