About the Role

The Salvation Army London Village is seeking a skilled Community Nurse to join our Adult Day Program team. As a Community Nurse, you will play a vital role in providing high-quality care to seniors with Alzheimer's and related forms of dementia, as well as those with medical fragility.

Key Responsibilities
  • Provide care in accordance with administrative policies, the College of Nurses of Ontario "Standards of Nursing Practice" and the standards of Gerontological Nursing.
  • Assess and monitor the condition of participants; recognize and interpret signs/symptoms and immediately report concerns to caregivers.
  • Monitor nutritional status of participants and liaise with the caregiver, Program Supervisor, and ADP team to resolve concerns and ensure appropriate meals are ordered.
  • Provide direction, guidance and support to PSW staff and assist with distribution of refreshments/meals, assist with meal preparation and clean up as required and aid participants who require additional support during mealtime.
  • Perform lifts, transfers, transport and assist participants with mobility challenges.
  • Provide appropriate participant intervention when managing participant responsive behaviours and provide leadership and support to team during crisis or medical emergencies.
Requirements
  • Completed a two (2) year Community College diploma in nursing, degree preferred.
  • Current, valid registration as a Registered Practical Nurse with the College of Nurses of Ontario.
  • Current Gentle Persuasive Approach training.
  • Current First Aid & CPR Certification.
  • Current Safe Food Handling Certification.
  • Valid Foot Care certificate with recent experience.
Working Conditions
  • This is a permanent part-time position based on 7 hours per week with the possibility of additional casual relief hours.
  • Flexibility in scheduling is required to meet program requirements; schedule including days and weekends.
  • The Supervisor will set the hours of work and work schedule.
