Community Nurse

ParaMed is seeking a skilled Registered Practical Nurse (RPN) to join our team in providing high-quality care to patients in their homes. As a RPN, you will be responsible for delivering patient-centered care, working collaboratively with other healthcare professionals to ensure the best possible outcomes.
Key Responsibilities:- Conduct scheduled visits to patients in their homes to assess their needs and provide care.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Provide education and support to patients and their families to promote health and well-being.
- Collaborate with other healthcare professionals to ensure seamless care coordination.
- Document patient information and progress in a timely and accurate manner.
- Registration with the College of Nurses of Ontario (CNO) as a Registered Practical Nurse in good standing.
- Valid driver's license and access to a reliable vehicle.
- Experience in delivering community care is an asset.
