We are seeking a highly skilled and compassionate Case Manager to join our team at the Canadian Mental Health Association. As a Case Manager, you will play a critical role in supporting individuals with mental health challenges and involvement in the criminal justice system.
Job Summary:The Case Manager will work collaboratively with individuals, their families, and community agencies to develop and implement personalized plans to address their mental health needs and reduce the likelihood of further conflict with the law.
Key Responsibilities:- Conduct comprehensive assessments to identify individual strengths and needs
- Develop and implement individualized plans to address mental health needs and reduce conflict with the law
- Collaborate with community agencies and healthcare providers to ensure seamless service delivery
- Provide education and support to individuals and their families to promote recovery and independence
- Monitor and evaluate the effectiveness of individualized plans and make adjustments as needed
- Master's degree in a related field (e.g. social work, psychology, nursing)
- Minimum 2 years of experience working in a community-based mental health setting
- Knowledge of the Ontario Mental Health Act and mental health reform principles
- Ability to work collaboratively with individuals, families, and community agencies
- Strong communication and interpersonal skills
