At CarePartners, we are committed to providing high-quality home health care services. We are seeking a skilled Physiotherapist to join our dynamic Rehabilitation Team in the role of Community Health Specialist.
About the RoleWe offer a competitive salary of $85,000 - $110,000 per year, comprehensive health and dental benefits, flexible scheduling, and opportunities for professional growth. As a Physiotherapist with CarePartners, you will provide critical support to patients in their homes and communities, conducting thorough assessments and developing effective care plans. You will work collaboratively with an interdisciplinary care team to ensure patients receive the best possible outcomes.
Responsibilities- Conduct thorough assessments of patients in their homes and communities
- Develop and maintain effective care plans in collaboration with the care team
- Provide one-on-one therapeutic support to clients, promoting independence and empowering them to achieve their goals
- Work independently and as an integral member of the care team
To be successful in this role, you will need:
- A Master's Degree (or equivalent) in Physiotherapy from a recognized university
- Registration with the College of Physiotherapists of Ontario
- Proven computer skills, including email, word processing, and work scheduling systems
- Strong assessment skills and the ability to participate in effective treatment programs
