Vancouver Coastal Health is seeking a skilled Social Worker 2 to join our team in Vancouver, BC. As a Social Worker 2, you will play a vital role in providing comprehensive social and psycho-social assessments, working from a harm reduction, culturally safe, trauma-informed, recovery-oriented, and client-centered model of care.
Key Responsibilities- Conduct comprehensive social and psycho-social assessments to identify clients' needs and develop effective care plans.
- Collaborate with interdisciplinary teams to facilitate transitions and provide consultative and direct services to clients, families, and community health professionals.
- Prepare and maintain accurate client records in accordance with established standards, policies, and procedures.
- Use health management and self-care management principles to identify problems, address issues, and variances from the plan of care.
- Establish and maintain therapeutic relationships based on respect and empathy.
- Current full registration with the British Columbia College of Social Workers.
- Master's Degree in Social Work from an accredited School of Social Work.
- Two years of experience working in a healthcare setting providing assessment and treatment services to individuals with complex mental health, substance use, and chronic medical concerns.
- Knowledge of the principles of recovery in mental illness.
- Knowledge of Psychosocial Rehabilitation methodology.
- Knowledge of the Mental Health Act and its application to client rights and staff obligations.
- Knowledge of community, social, and mental health resources.
- Knowledge and experience in providing culturally safe and competent care, particularly for Indigenous peoples.
- Demonstrated ability to identify emotional, social, or environmental barriers and intervene to eliminate stigma.
- Demonstrated ability to effectively manage conflict situations and aggressive behavior using non-violent crisis intervention and behavioral management techniques.
- Demonstrated ability to work independently and collaboratively as a member of an interdisciplinary team.
- Demonstrated ability to communicate effectively, both verbally and in writing, with clients, families, team members, and community agency personnel.
