About the Role
Vancouver Coastal Health is seeking a skilled Social Worker to join our Substance Use Intensive Case Management Team in Vancouver, BC. As a Social Worker with our organization, you will work collaboratively with an interdisciplinary team to provide comprehensive social and psycho-social assessments, conduct individual, family, and group counseling services, and facilitate transitions through the continuum of services for clients with complex discharge and/or intake/referral needs.
Key Responsibilities
- Conduct comprehensive social and psycho-social assessments to identify client needs and develop effective treatment plans.
- Collaborate with clients, families, and interdisciplinary teams to provide individual, family, and group counseling services.
- Facilitate transitions through the continuum of services for clients with complex discharge and/or intake/referral needs.
- Provide consultative and direct services to clients, families, staff, and community health professionals regarding issues such as adult guardianship, advanced directives, domestic violence, abuse, and/or neglect, immigration, housing, financial concerns, or matters concerning government agencies and authorities.
- Prepare and maintain client records in accordance with established standards, policies, and procedures.
Requirements
- Master's Degree of Social Work from an accredited School of Social Work.
- Three (3) years experience working in a psychiatric setting providing treatment services to individuals with complex mental health, addiction, and chronic medical concerns or equivalent.
- Valid BC Drivers License. Current full registration with the British Columbia College of Social Workers.
