Job Summary
Vancouver Coastal Health is seeking a skilled Social Worker to join the Kitsilano Mental Health team in Vancouver, BC. As a Social Worker with VCH, you will work collaboratively with an interdisciplinary team to provide comprehensive social and psycho-social assessments, conduct individual, family, and group counseling services, and facilitate transitions through the continuum of services for clients with complex needs.
Key Responsibilities
- Conduct comprehensive social and psycho-social assessments of clients
- Collaborate with clients and the interdisciplinary team to develop and implement treatment plans
- Provide individual, family, and group counseling services to clients
- Facilitate transitions through the continuum of services for clients with complex needs
- Attend clinical/case conferences as a member of the interdisciplinary team
- Provide consultative and direct services to clients, families, staff, and community health professionals
Qualifications
Education and Experience
- Master's Degree in Social Work from an accredited School of Social Work
- Current full registration with the British Columbia College of Social Workers
- Three (3) years of experience working in a psychiatric setting providing treatment services to individuals with complex mental health, addiction, and chronic medical concerns
Knowledge, Skills, and Abilities
- Knowledge of the principles of recovery in mental illness
- Knowledge of Psychosocial Rehabilitation methodology
- Knowledge of the Mental Health Act as it applies to the rights and obligations of clients and staff
- Knowledge of community, social, and mental health resources
