Job Summary
Vancouver Coastal Health is seeking a Social Worker to join the Primary Care team at Raven Song Community Health Centre in Vancouver, BC.
Key Responsibilities
- Provide clinical and consultative social work services to individuals with chronic health issues, mental health issues, poverty, and victims of violence.
- Conduct comprehensive social and psycho-social assessments, prepare comprehensive case histories, and provide psycho-education and interventions.
- Establish, maintain, and enhance therapeutic relationships, evaluate clients' response to treatment plans, and coordinate individualized goal-oriented treatment.
- Develop and enhance community communications/relationships, provide consultation services, and engage in on-going team planning, development, evaluation, and quality improvement activities.
Qualifications
- Current full registration with the British Columbia College of Social Workers.
- Master's degree in Social Work from an accredited university.
- Two (2) years' recent related experience working with individuals with mental health issues in a healthcare setting or an equivalent combination of education, training, and experience.
