Active Treatment Facilitator

The Delaware Department of Health and Social Services (DHSS) is seeking a highly skilled Active Treatment Facilitator to join our team. As an Active Treatment Facilitator, you will play a critical role in providing active treatment and assistance with activities of daily living to intermediate care residents.
Key Responsibilities- Provide active treatment and assistance with activities of daily living to intermediate care residents.
- Conduct periodic rounds to ensure the safety and well-being of clients.
- Implement treatment plans through individual and group therapeutic sessions.
- Update written records of therapeutic sessions, noting progress, problems, and possible solutions.
- Intervene in crisis situations, using appropriate therapeutic techniques.
- Observe and record any unusual behavior or significant behavior changes.
- Supervise clients to ensure their safety and security.
- Plan and conduct individual and group social activities, both on and off campus.
- Collaborate with public and private sector organizations to define and establish employment opportunities for clients.
- Participate in interdisciplinary team meetings to provide input on client needs, strengths, and weaknesses.
- Knowledge of instructing and assisting clients in developing functional, social, recreational, and vocational skills.
- Knowledge of working with persons with disabilities.
- Knowledge of applying laws, rules, regulations, standards, policies, and procedures.
- Knowledge of narrative report writing.
This position requires a satisfactory criminal background check and pre-employment drug testing. The State of Delaware participates in E-Verify and will provide the federal government with your Form I-9 information to confirm that you are authorized to work in the U.S.
