About the Role

The Department of Health and Social Services (DHSS) is seeking a skilled Active Treatment Facilitator to join our team. As a key member of our team, you will provide active treatment and assistance with activities of daily living to intermediate care residents.

Key Responsibilities

  • Provide active interaction and assistance with feeding, toileting, bathing, socialization, safety, personal hygiene, and ambulation.
  • Conduct periodic rounds to ensure the safety and well-being of clients.
  • Support residents in fulfilling treatment and learning plans developed by professional staff.
  • Implement treatment plans through individual and group therapeutic sessions.
  • Update written records of therapeutic sessions, noting progress, problems, and possible solutions.
  • Intervene in crisis situations, using appropriate therapeutic techniques.
  • Observe, record, and report any unusual behavior or significant behavior changes.
  • Supervise clients to ensure their safety and security.
  • Plan and conduct individual and group social activities, both on and off campus.
  • May work with public and private sectors to define and establish employment opportunities for clients.
  • Participate in interdisciplinary team meetings to provide input on client needs, strengths, and weaknesses.

Requirements

  • Knowledge of instructing and assisting clients in developing functional, social, recreational, and vocational skills.
  • Knowledge of working with persons with disabilities.
  • Knowledge of applying laws, rules, regulations, standards, policies, and procedures.
  • Knowledge of narrative report writing.
