Job Summary

The Delaware Department of Health and Social Services is seeking a highly skilled Active Treatment Facilitator to join our team. As an Active Treatment Facilitator, you will play a critical role in providing active treatment and assistance with activities of daily living to intermediate care residents.

Key Responsibilities
  • Provide active treatment and assistance with activities of daily living to intermediate care residents.
  • Conduct periodic rounds to ensure the safety and well-being of clients.
  • Implement treatment plans through individual and group therapeutic sessions.
  • Update written records of therapeutic sessions noting progress, problems, and possible solutions.
  • Intervene in crisis situations using appropriate therapeutic techniques.
  • Observe and record any unusual behavior or significant behavior changes.
  • Supervise clients to ensure their safety and security.
  • Plan and conduct individual and group social activities on and off campus.
  • Collaborate with public and private sector organizations to define and establish employment opportunities for clients.
  • Participate in interdisciplinary team meetings to provide input on client needs, strengths, and weaknesses.
Requirements
  • Knowledge of instructing and assisting clients in developing functional, social, recreational, and vocational skills.
  • Knowledge of working with persons with disabilities.
  • Knowledge of applying laws, rules, regulations, standards, policies, and procedures.
  • Knowledge of narrative report writing.
Working Conditions
