Patient Care Coordinator

We are seeking a Patient Care Coordinator to join our team at VON Canada. As a key member of our Home Health and Visiting Care Team, you will provide personalized care and support to patients in their homes, promoting independence, dignity, and well-being.
About the Role:This is a permanent full-time position that involves working 80 hours bi-weekly. The ideal candidate will have a College Diploma or CEGEP Diploma in a related field and possess relevant experience in healthcare, with Continuing Care Assistant Certification being an asset.
Responsibilities:- Provide personal care and assistance with daily living activities, including bathing, dressing, grooming, and toileting
- Assist patients with mobility, transfers, and positioning
- Prepare and serve meal trays, maintaining patient nutrition and hydration needs
- Conduct vital signs, administer first aid when necessary, and maintain accurate records
- Maintain patient confidentiality and adhere to organizational policies and procedures
- College Diploma or CEGEP Diploma in a related field (e.g., Continuing Care Assistant, Personal Support Worker)
- Relevant experience in healthcare, preferably in home care or visiting care settings
- Continuing Care Assistant Certification, First Aid Certificate, CPR Certificate, and Nursing Aide Certificate an asset
- Criminal Record Check, Child Abuse Registry Check, Vulnerable Sector Check, and Tuberculosis Test required
- Ability to work independently, prioritize tasks, and manage time effectively
- A competitive salary range of $43,000 - $53,000 per year, commensurate with experience

The estimated annual salary for this role is approximately $47,500, based on a 38-hour workweek and assuming a mid-point within the given salary range. Please note that actual salaries may vary depending on individual circumstances and collective agreements.
