**About the Opportunity**:
Reporting to the Zone Director, the Manager of Care Coordination is accountable to foster an environment of learning, respect and accountability through personal leadership, coaching and annual performance reviews with all direct reports resulting in a sustainable healthy workplace; lead and direct the operations of the Continuing Care program that is aligned with the priorities of Nova Scotia Health and provincial priorities, legislation, policy and standards; foster a collaborative approach to internal and external relationships with stakeholders, partners and service providers within central zone and across the province in order to meet the continuing care health needs of Nova Scotians; accountable to manage within allocated budgets.
**About You**:
We would love to hear from you if you have the following:
- Baccalaureate degree in a healthcare field and a minimum of 5 years recent and related leadership/management experience is required
- Must be currently registered with an applicable regulatory body
- Demonstrated leadership skills in a healthcare environment and ongoing professional development
- Progressive experience in management and administration in a health care/ community setting, including financial and human resource management
- Demonstrated commitment to a philosophy of independence for those who require care and to the utilization of evidence to inform practice
- Demonstrated ability to build and maintain effective collaborative relationships and partnerships with stakeholders and clients
- Strong human resource management, financial management, interpersonal, and communication skills
- Excellent organizational, time management and problem solving skills
- Experience identifying trends, preparing reports, and implementing change, leadership, strategic thinking, and analytical skills
- Demonstrated planning, organization and time management skills. Ability to deal with competing priorities
- Knowledge of applicable legislation and regulations including the NS Health Authorities Act, Homes for Special Care Act, Homemakers Services Act, etc
- Knowledge of applicable policies and standards including the NS Department of Seniors and Long Term Care (DSLTC), continuing care policies and standards, guidelines, etc
- Demonstrated knowledge and understanding of the NS_ Action for Health_ & LEADS in a Caring Environment
- Experience in managing in a unionized environment
- Competencies in other languages an asset; French preferred

**Hours**:

**Compensation and Benefits**:
$42.94 - $57.05 hourly
