About the Opportunity

Our Healthcare Team is Seeking a Dedicated Professional

The Patient Care Navigator Professional plays a vital role in our interdisciplinary healthcare team, providing collaborative and holistic care on the primary to quaternary care continuum.

Social determinants of health are integral to this role, focusing on psychosocial, emotional, and spiritual wellbeing through engagement, assessment, planning, intervention, and evaluation.

This dynamic and interactive process involves building upon patients'/clients' strengths by focusing on existing coping strategies, problem-solving skills, past successes, and personal resources.

We are looking for a professional with a Bachelor's degree in Social Work from a recognized university and current registration with the Nova Scotia College of Social Workers.

A minimum of 2 years' post-Bachelor of Social Work related clinical experience is required, with previous palliative care experience being an asset.

Grief & Bereavement Training and experience working in a community-based setting are also preferred.

The ideal candidate will have demonstrated ability to use a client-/family-centered approach, complete biopsychosocial assessments, and possess excellent communication skills.

The successful candidate will be able to assist patients and families with navigation of local community resources and agencies.

Travel to other sites may be required, and having access to a reliable vehicle is essential.

Competencies in other languages, particularly French, are considered an asset.
