Nurse Case Manager

Vancouver Coastal Health is seeking a skilled Nurse Case Manager to join our Home Health team at Pender Community Health Centre in Vancouver, BC.
Key Responsibilities- Manage a caseload of clients with complex health care needs, providing continuity of care across the continuum.
- Conduct assessments, identify problems, and address issues or variances from the plan of care in collaboration with the interdisciplinary team.
- Develop and maintain formal General Practitioner Partnerships as appropriate and in accordance with CHA service delivery model.
- Act as a clinical resource for the interdisciplinary team, providing information based on nursing theory and practice related to clients with complex health care needs.
- Consult, confer, and collaborate with multidisciplinary team members, physicians, educational resources, and other healthcare professionals to achieve excellence in client-centred education and research.
Home Health programs provide a range of healthcare and support services to people living in the community who have acute, chronic, palliative, or rehabilitative healthcare needs.
Qualifications- Current practicing registration as a Registered Nurse or a Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Three (3) years' recent clinical nursing experience working in acute care with a medical and/or geriatric focus, recent case management or home care nursing experience, or an equivalent combination of education, training, and experience.
- Local area travel requires the use of a personal vehicle. Valid BC Driver's License.
