Job Summary
Vancouver Coastal Health is seeking a Nurse Case Manager to join their team at Lions Gate Hospital in North Vancouver, BC.
Key Responsibilities:
- Manage a caseload of patients requiring assisted living services.
- Conduct comprehensive assessments to determine client goals and needs.
- Develop and implement individualized care plans to meet client needs.
- Collaborate with interdisciplinary teams to provide continuity of care.
- Provide education and support to clients and their families.
Qualifications:
- Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Three (3) years' recent clinical nursing experience in Home Health and case management or an equivalent combination of education, training and experience.
- Valid BC Driver's License.
