Vancouver Coastal Health (VCH) is seeking a dedicated Nurse Case Manager to enhance our Home Health team.
In this role, you will be responsible for:
- Overseeing a designated group of clients with chronic illnesses or long-term physical disabilities.
- Applying principles of chronic disease management and self-care to perform assessments, identify challenges, and collaborate with physicians and interdisciplinary team members to address care plan variances.
- Ensuring continuity of care by coordinating clinical interventions, evaluating outcomes based on client goals, and facilitating transitions.
- Establishing and maintaining partnerships with General Practitioners as per the service delivery model.
- Serving as a clinical resource for the interdisciplinary team, offering insights based on nursing theory and practice for clients with complex health needs.
- Collaborating with multidisciplinary team members, physicians, and other healthcare professionals to promote excellence in client-centered education and research.
About Home Health Programs:
Home Health programs deliver a variety of healthcare and support services to individuals in the community facing acute, chronic, palliative, or rehabilitative health challenges.
Client Demographics:
Clients typically experience ongoing health issues and/or complex personal or social challenges, necessitating careful care planning and resource coordination. This population includes frail elderly individuals, younger clients with disabilities, and adults with chronic conditions requiring support to maintain or enhance their independence.
Qualifications
Education & Experience
- Active registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- A minimum of three (3) years of recent clinical nursing experience in acute care with a focus on medical and/or geriatric care, along with recent case management or home care nursing experience, or an equivalent combination of education and experience.
- Local travel may require the use of a personal vehicle and a valid BC Driver's License.
Knowledge & Skills
- Ability to apply critical thinking and sound judgment in problem-solving within safe client care limits.
- Proficiency in physical assessment and chronic disease management.
- Competence in psychomotor skills relevant to nursing practice.
- In-depth understanding of BCCNM standards of practice and clinical guidelines.
- Capability to work independently and prioritize care for a caseload of clients.
- Comprehensive knowledge of various health disciplines and their roles in client care.
- Ability to collaborate effectively as part of an interdisciplinary team.
- Familiarity with adult education principles and methodologies.
- Skill in utilizing research and methodologies in practice.
- Leadership capabilities to guide and direct others.
- Proficiency in facilitating group interactions involving professionals and clients.
- Effective communication skills, both oral and written.
- Ability to negotiate and manage relationships with co-workers, physicians, clients, and external agency staff.
- Competence in conflict resolution and adaptability to unexpected situations.
- Skill in conducting comprehensive assessments, including physical, mental, and psychosocial evaluations.
- Physical capability to perform the duties associated with this position.
- Computer literacy to manage patient care information systems and communication tools.
