Patient Care Coordinator

We are seeking a motivated and organized Patient Care Coordinator to support and lead our nursing team at the Kootenay Boundary Regional Hospital in Trail, B.C.
Job Summary:The Patient Care Coordinator practices in accordance with the standards of professional practice and code of ethics as outlined by the British Columbia College of Nurses and Midwives (BCCNM) as well as within a patient and family centered care model.
Key Responsibilities:- Coordinate day-to-day operations and quality patient care within a designated clinical area(s)
- Support the seamless movement of patients through the healthcare system and in accessing appropriate resources
- Supervise and serve as a role model and a resource to staff for clinical decision-making, clinical assessments of patients and in the counseling and education of families
- Review, analyze and make recommendations on the development of administrative systems such as policies, procedures and standards of patient care, adequate staffing levels, and a continuous quality improvement environment
- Liaise with the interdisciplinary team for identification, coordination and efficient utilization of resources as well as plans for staff educational needs
