Patient Care Coordinator

The Registered Nurse plays a vital role in providing high-quality patient care and education. Key responsibilities include:
- Providing diabetes education and support to patients, and participating as a member of the interdisciplinary team.
- Utilizing strong understanding of adult education principles and change facilitation skills, in addition to excellent working knowledge of diabetes care and management, including Type 1, Type 2, gestational diabetes, and pediatric diabetes.
- Enhancing quality of life for patients and their families by consulting, conferring, and collaborating with other healthcare providers.
- Demonstrating continuous improvement and evidence-based practice in nursing care.
- Maintaining and advancing own clinical competence.
- Participating on designated hospital committees and approved research projects as assigned.
This is a casual opportunity, which means hours are not guaranteed and may vary, with shifts scheduled on short notice. Casual employees are eligible to apply for internal postings, including permanent and temporary positions.
Shifts of work will be scheduled during the day, rotating from 8:30 to 16:30 hours.
