Mental Health Social Worker

We are seeking a skilled Mental Health Social Worker to join our team at Chatham-Kent Community Health Centres. This is a full-time position offering a competitive salary and comprehensive benefits package.
About the RoleThe successful candidate will work in a dynamic healthcare setting, providing direct care to patients and collaborating with other professionals to develop and implement treatment plans.
Responsibilities- Conduct assessments and provide counseling services to individuals, families, and groups.
- Develop and implement individualized treatment plans that address mental health concerns and promote overall well-being.
- Collaborate with other healthcare professionals to ensure comprehensive care and effective communication.
- Maintain accurate records and reports, ensuring confidentiality and adherence to regulatory requirements.
To be considered for this role, you must possess:
- A Master's degree in Social Work from an accredited program.
- A minimum of 2 years of experience working in a mental health setting.
- Membership in a provincial association of social workers.
- Certification in crisis intervention and conflict resolution.
This is a site-based position, requiring regular attendance at our health centre location. The successful candidate must be able to work independently and as part of a team, demonstrating flexibility and adaptability in a fast-paced environment.
