Job Overview

We are seeking an experienced Licensed Practical Nurse (LPN) to join our team as a Wellness Nurse. In this role, you will be responsible for promoting the overall well-being of our residents, ensuring their health and wellness needs are met, and providing exceptional care and support.

Responsibilities
  • Conduct monthly wellness visits for all residents, identifying their ongoing needs and services to promote the highest quality resident services.
  • Assist the Resident Care Director in conducting resident evaluations, gathering information at time of move-in, 30 days post-move-in, semi-annually, and with any change in status.
  • Monitor and follow up with residents who have experienced falls, accidents, or incidents, documenting and reporting as required and directed.
  • Maintain communications with residents' families and/or responsible parties to ensure medical needs are being met.
  • Contact attending physicians when necessary and/or upon family request.
  • Evaluate and document weights and vital signs monthly for each resident.
Requirements
  • A minimum of one year of experience in home health, assisted living, hospital, or long-term care environment.
  • Graduate of an approved college/school of nursing.
  • Maintains a current state license as a professional LPN/LVN.
  • Demonstrates knowledge of nursing practices, techniques, and methods applied to health and wellness resident care and federal, state, and local regulations.
Benefits

We offer a competitive salary ranging from $ $55,000 to $65,000 per year, depending on location and experience. Additionally, we provide:
