Job SummaryThe Clinical Care Specialist - Assisted Living will be responsible for conducting monthly wellness visits for all residents, monitoring and following up on any falls or incidents, documenting resident information, maintaining communication with family members, and ensuring that medical needs are met. You'll also assist in maintaining wellness files according to Sunrise policies and federal regulations.
Responsibilities & Qualifications- Resident Care: Identify ongoing needs and services of residents, ensure proper follow-through, and participate in evaluations and documentation.
- Data Collection: Assist in conducting resident evaluations and data gathering at time of move-in, 30 days post-move-in, semi-annually, and with any change in status.
- Wellness Visits: Conduct monthly wellness visits for all residents, monitor vital signs, and document findings.
- Communication: Maintain open communication with family members and caregivers to ensure medical needs are met.
- Maintenance: Ensure weights and vital signs are obtained monthly for each resident, maintain medical supplies, and emergency kits.
- Licensure: Current state license as a professional Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN).
- Experience: Minimum one year experience in home health, assisted living, hospital, or long-term care environment.
- Skills: Knowledge of nursing practices, techniques, and methods applied to health and wellness resident care, federal, state, and local regulations, good assessment skills, and ability to handle multiple priorities.
The estimated salary for this role is $55,000 - $65,000 per year, depending on location and qualifications.
