Job Title: Case Manager/ Case Worker

Job Summary:

We are seeking a skilled Case Manager/ Case Worker to join our team at Good Shepherd Centres. As a key member of our team, you will provide respectful and appropriate housing support services to our clients, extending our mission to residential and non-residential clients.

Key Responsibilities:
  • Develop and maintain a caseload of clients, providing individualized support and guidance to help them achieve their goals.
  • Conduct intakes and assessments to identify client needs and develop personalized plans to address them.
  • Design and implement interview schedules to evaluate the effectiveness of our programs and services.
  • Maintain confidentiality and adhere to strict data protection protocols.
Requirements:
  • A post-secondary education in a relevant field, such as social work or counseling.
  • Experience working in a residential setting, preferably with vulnerable populations.
  • Strong knowledge of issues impacting cis women, trans, and non-binary individuals, including violence, abuse, mental illness, homelessness, poverty, and substance use.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • CPR certification and a valid Ontario driver's license (G2) are assets.
