Job Title: Mental Health Support Worker

We are seeking a skilled Mental Health Support Worker to join our team at Heritage Nanny Community Living and Support Inc. The successful candidate will provide support and guidance to clients with mental health issues, helping them to achieve their goals and improve their overall well-being.

Key Responsibilities:
  • Prepare reports and case histories to document client progress and needs
  • Administer and coordinate the delivery of specific services within the community
  • Assess client's relevant skill strengths and development needs
  • Develop and implement rehabilitation programs to support clients in achieving their goals
  • Provide crisis intervention and suicide prevention services
  • Supervise activities of clients living in group homes and half-way houses
  • Liaise with other social services agencies and healthcare providers involved with clients
  • Develop and maintain program statistics for purposes of evaluation and research
Requirements:
  • 2 years to less than 3 years of experience in a related field
  • Excellent interpersonal and communication skills
  • Ability to work independently and as part of a team
  • Flexibility and adaptability in a fast-paced environment
