Social Worker

We have an exciting opportunity for a social worker to join our primary care team at Northern Health Authority. As a social worker in primary care, you will play a vital role in providing comprehensive assessment, care planning, and intervention to individuals and families. You will work collaboratively with other healthcare providers and the interprofessional team to provide direct psychosocial care, counseling, and group work, as well as system navigation and advocacy. Key Responsibilities
• Provide comprehensive assessment and care planning for individuals and families
• Develop and implement individualized care plans to meet the unique needs of patients
• Collaborate with other healthcare providers and the interprofessional team to provide direct psychosocial care and counseling
• Provide system navigation and advocacy to support patients and families in accessing community resources and services
• Participate in quality improvement initiatives to enhance patient care and outcomes

Requirements
• Bachelor's degree in social work from an accredited university
• Two years of recent experience in a healthcare setting or a combination of education, training, and experience
• Current registration with the British Columbia College of Social Workers
• Current valid BC driver's license

• Knowledge integration: ability to integrate best practice and current research evidence to support professional practice decisions and actions
• Communication: ability to communicate effectively with people and their families, as well as the public, physicians, and members of the interdisciplinary team
• Critical thinking: ability to problem-solve and demonstrate critical thinking and decision-making skills
• Human caring and relationship-centered practice: ability to promote person-centered care, including maintaining sensitivity to diverse cultures and preferences, social justice concerns, and the need for advocacy
• Management: ability to organize work, set objectives and priorities, and achieve results
• Leadership: ability to demonstrate creative planning for change and innovation, and participate in professional development of self and others
• Teamwork: ability to foster teamwork and a commitment to excellence in the provision of person-centered care, promoting cooperation among the interprofessional team and collaborating across disciplines
• Equipment: ability to operate a computer and other related equipment as required in the specific practice area
• Ability: physical ability to perform the duties of the position
